Restrictions

The following forms are to be completed by the applicant. Illegible and incomplete forms will not be processed. Proof of identification must be provided before a report will be issued. Those who are representatives of an organization shall provide proof of the organization they represent. All fees for records must be paid before reports will be released.

Request for Police Report

The Request for Police Report Form is utilized when requesting copies of an automobile accident, crime report, domestic incident, or any other department incident report.

Request for Criminal History & Records Check


The Request for Criminal History Form is used primarily by employers who require a criminal history check. Those who are in the process of adopting children also use this form.

Permit Request for Size, Weight & Load Movement

Any load that is moved upon township roadways that exceeds the weight limit or width restrictions must receive special permission to affect the move. Applicants will pay the total costs of escorting the movement and are responsible for any costs of damages to public and private property that occur during movement. All applicants must comply with township, county, and Commonwealth of Pennsylvania regulations. Download the Oversize Load Application to apply for a movement permit, which must be carried in the permitted vehicle at all times.

Identity Theft

To make certain that you do not become responsible for any debts incurred by an identity thief, you must prove that you didn't create the debt to each of the companies where accounts were opened or used in your name. A group of credit granters, consumer advocates, and attorneys at the Federal Trade Commission (FTC) developed an Identity Theft Affidavit to make it easier for fraud victims to report information. While many companies accept this affidavit, others require that you submit more or different forms. Before you send the affidavit, contact each company to find out if they accept it.

The Federal Bureau of Investigation: The Internet Crime Complaint Center is to provide the public with a reliable and convenient reporting mechanism to submit information to the Federal Bureau of Investigation concerning suspected Internet-facilitated criminal activity and to develop effective alliances with law enforcement and industry partners. Information is analyzed and disseminated for investigative and intelligence purposes to law enforcement and for public awareness. You can file a complaint online at www.ic3.gov

Alarm Registration

Property owners are required to submit an Alarm Registration Form with the department to register their alarms.

Solicitors Permit

Local ordinance requires all solicitors to submit a Solicitors Permit Application prior to soliciting within the township. Residents can also submit a No Solicitation Request to be put on the Police Department's No Soliciting List.

Vacation Notice Form

Please complete the Vacation Notice Information Form indicating the dates of your vacation and the information will be forwarded to the Patrol Division. Please remember to properly secure your residence and keep the area well lit with lights on timers. If you plan to be away for an extended period of time it is suggested that you cancel all deliveries and suspend your mail service.

Voluntary Statement

The Upper Uwchlan Township Police Department investigates all reported crimes. Provide a detailed account of the matter you wish to report by submitting a Voluntary Statement.

Police Services Survey

The Upper Uwchlan Township Police Department is a service oriented organization providing quality police services to the residents of our community. Police Department management randomly selects police incidents for review and evaluation. This evaluation is accomplished using the Police Services Survey. Feedback from randomly selected citizens is used to evaluate our service delivery. If selected, please take the time to participate. Thank you for the time and effort needed to complete the survey.
